Nurse Case Manager RN - $10,000 Sign-On Bonus

Case Manager RN – Full-Time (Permanent)
Location: Tallahassee, FL 32310
Schedule: Day Shift (8:00 AM – 4:30 PM) 1 Weekend every 4 weeks

Compensation: $31.94/hr – $43.92/hr (based on experience) + $10,000 Sign-On Bonus + Relocation Assistance

Key Responsibilities

• Conduct comprehensive clinical & psychosocial assessments
• Develop and implement individualized care plans
• Coordinate with interdisciplinary teams
• Perform utilization reviews & ensure medical necessity
• Lead discharge planning & remove care barriers
• Collaborate with physicians, families & external agencies
• Ensure compliance with hospital & regulatory standards

Qualifications

Education:
• ADN required | BSN preferred

Experience:
• 3+ years Case Management experience in acute care
OR
• 3+ years in Med/Surg, ICU, ED, Tele, PCU, or Neuro

(Home Health/Insurance background welcome with strong acute care experience)

Licensure:
• Active Florida RN License (or Compact; FL required within 90 days)

Compensation

• $31.94/hr – $43.92/hr (based on experience)
• Evening Diff: +$2.50/hr
• Weekend Diff: +$2.00/hr

$10,000 Sign-On Bonus (paid over 2 years)
