Salary: $85,000 – $95,000 annually
Additional Compensation:

  • Mileage Reimbursement: $0.65/mile
  • Cell Phone Reimbursement: $60/month

Job Summary

We are seeking a compassionate and experienced Hospice Admissions Registered Nurse (RN) to join our team in Denton County, TX. In this role, you will serve as the first clinical point of contact for patients and families entering hospice care, providing comprehensive assessments, coordinating admissions, and ensuring a smooth transition into hospice services.

Key Responsibilities

  • Conduct patient assessments to determine hospice eligibility.
  • Guide patients and families through the hospice admission process with compassion and professionalism.
  • Develop individualized plans of care for newly admitted patients.
  • Collaborate with the interdisciplinary care team to coordinate patient transitions.
  • Provide skilled nursing care in accordance with hospice care plans.
  • Maintain accurate, timely, and compliant clinical documentation.
  • Participate in a rotating on-call schedule.
  • Perform additional clinical and administrative duties as assigned.

Requirements

  • Active Texas Registered Nurse (RN) License required.
  • Minimum 1 year of Hospice or Senior Care experience required.
  • Strong knowledge of hospice philosophy, eligibility criteria, and end-of-life care.
  • Excellent communication, assessment, and interpersonal skills.
  • Ability to work independently and effectively manage daily responsibilities.
  • Flexible and adaptable to changing patient needs.
  • Valid driver's license, reliable transportation, and current auto insurance.
  • Must be 21 years of age or older.
  • Successful background check and professional references required.

Schedule

  • Full-Time
  • Monday – Friday
  • 8:00 AM – 5:00 PM
  • Rotating on-call responsibilities
  • Travel throughout assigned territory
