RN Case Manager | Permanent Full-Time | Day Shifts

Job Title: RN Case Manager – Acute Care Hospital

Job Summary:

RN Case Manager coordinates patient care across the hospital. You will help plan treatment, support safe discharge, work with doctors and families, and connect patients with needed services like rehab or home care.

Compensation:

• $34/hr - $44.00/hr (Depends on Experience)
• $1.50/hr weekend differential
• Sign-on & relocation: case-by-case basis.

Shift Details

• Day shifts only (with rotating weekends)

Requirements:

• Active RN license.
• Associate Degree or Nursing Diploma minimum
• 2+ years recent acute care Case Management RN experience (within last year)
• Strong communication, assessment, and coordination skills
