Family Nurse Practitioner (FNP) - Primary Care

Qualifications:

• Master's or Doctorate degree from an accredited Nurse Practitioner program.
• Current, unrestricted California Nurse Practitioner license.
• Board Certification through the California Board of Registered Nursing.
• Active, unrestricted DEA license.
• Current BLS certification.
• Minimum of 2 years of Primary Care clinical experience required.
• Experience in a Community Health Center, Federally Qualified Health Center (FQHC), or underserved healthcare setting preferred.
• Excellent clinical assessment, communication, and patient education skills.
• Ability to work collaboratively within a multidisciplinary healthcare team.

Key Responsibilities:

• Deliver comprehensive primary care services including health assessments, diagnosis, treatment, and preventive care.
• Perform physical examinations and obtain complete patient medical histories.
• Diagnose acute and chronic medical conditions and develop individualized treatment plans.
• Prescribe and manage medications while monitoring patient progress.
• Educate patients on disease prevention, healthy lifestyle choices, and medication adherence.
• Coordinate referrals and collaborate with specialists to ensure continuity of care.
• Work closely with physicians, behavioral health providers, nurses, and other healthcare professionals in a team-based care model.
• Maintain accurate and timely documentation within the electronic health record (EHR).
• Participate in provider meetings, cross-coverage, and after-hours on-call rotation as needed.
• Support quality improvement initiatives and deliver compassionate, culturally responsive patient care.
