Job Summary:

Qualifications:
- Master's or Doctorate degree from an accredited Nurse Practitioner program.
- Current, unrestricted California Nurse Practitioner license.
- Board Certification through the California Board of Registered Nursing.
- Active, unrestricted DEA license.
- Current BLS certification.
- Minimum of 2 years of Primary Care clinical experience required.
- Experience in a Community Health Center, Federally Qualified Health Center (FQHC), or underserved healthcare setting preferred.
- Excellent clinical assessment, communication, and patient education skills.
- Ability to work collaboratively within a multidisciplinary healthcare team.

Key Responsibilities:
- Deliver comprehensive primary care services including health assessments, diagnosis, treatment, and preventive care.
- Perform physical examinations and obtain complete patient medical histories.
- Diagnose acute and chronic medical conditions and develop individualized treatment plans.
- Prescribe and manage medications while monitoring patient progress.
- Educate patients on disease prevention, healthy lifestyle choices, and medication adherence.
- Coordinate referrals and collaborate with specialists to ensure continuity of care.
- Work closely with physicians, behavioral health providers, nurses, and other healthcare professionals in a team-based care model.
- Maintain accurate and timely documentation within the electronic health record (EHR).
- Participate in provider meetings, cross-coverage, and after-hours on-call rotation as needed.
- Support quality improvement initiatives and deliver compassionate, culturally responsive patient care.
