Responsibilities:

• Provide direct patient care, including assistance with activities of daily living, such as bathing, dressing, and grooming

• Monitor and report patient conditions, including vital signs and changes in behavior or physical status

• Collaborate with nursing staff to develop and implement individualized care plans

• Maintain accurate and detailed records of patient care and treatments

• Participate in the continuous quality improvement process, identifying areas for improvement and suggesting innovative solutions

• Recognize and respond to emergency situations, such as code blue or falls
