Able Home Healthcare is looking for a passionate and dedicated Home Health Physical Therapist to join our growing team in San Diego, CA. Responsibilities:

• Conduct comprehensive in-home physical therapy evaluations and develop individualized treatment plans
• Provide skilled PT interventions including therapeutic exercise, gait training, and neuromuscular re-education
• Monitor and document patient progress, adjusting care plans as clinically appropriate
• Collaborate with physicians, nurses, and interdisciplinary care team members to ensure coordinated patient care
• Educate patients and caregivers on home exercise programs, fall prevention, and safe mobility techniques
• Maintain accurate and timely clinical documentation in compliance with Medicare and regulatory standards
• Manage a caseload of home health patients across the San Diego area with a flexible schedule

Requirements:

• Active California Physical Therapist (PT) license in good standing
• Graduate of an accredited Physical Therapy program (DPT or MPT)
• Home health or outpatient orthopedic/neurological experience preferred
• Strong knowledge of Medicare home health guidelines and documentation requirements
• Excellent communication, time management, and organizational skills
• Valid California driver's license and reliable personal vehicle with current auto insurance
• Compassionate, self-motivated, and comfortable working independently in a field-based setting
