About the Role:

• Provide personalized care and support to clients in their homes, focusing on their unique needs and preferences

• Utilize your expertise in Hoyer lifts to assist with safe transfers, ensuring the comfort and dignity of our clients.

• Collaborate with our interdisciplinary team to develop and implement personalized care plans that promote independence, comfort, and overall well-being

• Communicate effectively with clients, families, and healthcare professionals to ensure seamless care coordination

• Maintain accurate and timely records of client care and services, adhering to all regulatory and company standards

Responsibilities:

• Provide routine personal care, including bathing, grooming, and dressing

• Assist with activities of daily living (ADLs), such as toileting, transferring, and

• Monitor and report changes in client condition or behavior to Care Coordinators and healthcare providers

• Participate in ongoing education and training to stay current with industry best practices and regulatory requirements
