About the Role:
Able Home Healthcare in San Diego, CA is looking for a compassionate and dedicated Licensed Vocational Nurse (LVN) to join our home health team. Responsibilities:

• Provide skilled nursing care to home health patients in accordance with physician-approved care plans
• Administer medications, wound care, injections, and other treatments as prescribed
• Monitor and document patient vital signs, health status, and progress toward care goals
• Communicate effectively with patients, families, and interdisciplinary care team members
• Educate patients and caregivers on disease management, medications, and home safety
• Maintain accurate and timely clinical documentation in compliance with home health regulations
• Identify changes in patient condition and escalate concerns to the supervising RN or physician as needed

Requirements:

• Active California Licensed Vocational Nurse (LVN) license in good standing
• Previous home health, hospice, or community health nursing experience preferred
• Current CPR/BLS certification required
• Valid California driver's license and reliable personal transportation
• Strong clinical assessment and critical thinking skills
• Proficiency with electronic health records (EHR) and clinical documentation
• Compassionate, self-motivated, and able to work independently in a field-based setting
