Job Summary:

The Blossoms at Dierks Rehab & Nursing Center is seeking a dedicated and compassionate Certified Nursing Assistant to join our team of healthcare professionals. As a fundamental member of our care team, you will provide exceptional support to our residents, ensuring their physical, emotional, and spiritual needs are met with dignity and respect.

Responsibilities:

• Provide direct patient care under the supervision of licensed nurses, including vital signs, wound care, and medication administration

• Assist residents with activities of daily living (ADLs) such as bathing, dressing, grooming, and toileting

• Maintain accurate and up-to-date records of resident care and services

• Collaborate with interdisciplinary teams to develop and implement individualized care plans

• Participate in discharge planning and coordination with families and caregivers

• Maintain a clean and safe environment, adhering to our facility's infection control and safety standards

Requirements:

• Certified Nursing Assistant (CNA) certification or willingness to obtain within a specified timeframe

• High school diploma or equivalent

• Ability to lift up to 50 pounds and stand for long periods

• Excellent communication and interpersonal skills

• Flexibility to work varied shifts, including weekends and holidays

• Passion for delivering exceptional patient care and promoting a supportive environment
