Able Home Healthcare is looking for a passionate and skilled Home Health Occupational Therapist to join our growing team in San Diego, CA. Responsibilities:

• Conduct comprehensive occupational therapy evaluations and develop individualized treatment plans for home health patients
• Provide skilled OT interventions focused on ADLs, fine motor skills, cognitive rehabilitation, and functional independence
• Monitor and document patient progress, adjusting care plans as clinically appropriate
• Collaborate with physicians, nurses, and interdisciplinary care team members to ensure coordinated patient care
• Educate patients and caregivers on therapeutic techniques, adaptive equipment, and home safety modifications
• Maintain accurate and timely clinical documentation in compliance with Medicare and regulatory standards
• Manage a caseload of home visits throughout the San Diego area

Requirements:

• Master's or Doctoral degree in Occupational Therapy from an accredited program
• Current California OT license in good standing
• NBCOT certification required
• Home health experience preferred; new grads with clinical fieldwork in home health encouraged to apply
• Valid driver's license, reliable transportation, and current auto insurance
• Strong knowledge of Medicare home health guidelines and documentation requirements
• Excellent communication, time management, and organizational skills
• Compassionate, self-motivated, and comfortable working independently in the field
