About the Role:

As a Licensed Practical Nurse (LPN) at Rock River Nursing & Rehab, you will be responsible for providing direct patient care, administering medications, and collaborating with our interdisciplinary team to develop and implement care plans that meet the unique needs of each resident. You will also have the opportunity to build meaningful relationships with our residents, their families, and your colleagues, and contribute to a positive and supportive work environment.

Responsibilities:

• Provide direct patient care, including administering medications, wound care, and physical therapy interventions

• Collaborate with the interdisciplinary team to develop and implement individualized care plans

• Assist with admissions, transfers, and discharges, ensuring a smooth and efficient transition for our residents

• Participate in quality improvement initiatives and contribute to maintaining a high level of patient satisfaction and satisfaction

• Maintain accurate and detailed records of patient care, treatments, and medications administered

Requirements:

• Current LPN licensure in the state of Wisconsin

• High school diploma or equivalent required; associate's degree or higher in nursing or related field preferred

• Minimum one year of experience as an LPN, with experience in long-term care and/or rehabilitation settings a plus

Per Diem Shift:

We are seeking a flexible and adaptable individual to work a variety of shifts, including evenings, weekends, and holidays. As a per diem employee, you will have the opportunity to work on an as-needed basis, with varying shifts and schedules.
