Position: Family Nurse Practitioner (FNP) / Physician Assistant (PA)
Location: Southbury, Connecticut
Schedule: Full-Time | Permanent
A well-established, not-for-profit community healthcare organization is seeking a Family Nurse Practitioner (FNP) or Physician Assistant (PA) to join its outpatient primary care practice in Southbury, Connecticut. This is a full-time, permanent opportunity focused on delivering high-quality, patient-centered care in a collaborative clinical environment.
The ideal candidate will have a strong foundation in primary care, preventive medicine, and chronic disease management. Providers will care for a diverse patient population across the lifespan and manage a wide range of conditions, including diabetes, hypertension, asthma, infections, and other acute and chronic illnesses. Both experienced providers and new graduates are encouraged to apply.

Key Responsibilities• Provide comprehensive outpatient primary care services, including evaluation, diagnosis, and treatment
• Conduct routine physical examinations, preventive screenings, and health assessments
• Diagnose and manage acute and chronic conditions, including diabetes, hypertension, asthma, infections, and other medical concerns
• Order, review, and interpret diagnostic testing, including laboratory studies and imaging
• Prescribe medications and develop individualized treatment plans
• Educate patients regarding disease prevention, wellness, and lifestyle modifications
• Collaborate with physicians, specialists, and multidisciplinary healthcare teams to ensure coordinated patient care
• Maintain accurate and timely documentation within the electronic medical record (EMR) system
• Participate in quality improvement initiatives and population health programs
• Support the delivery of evidence-based, patient-focused healthcare services
For Nurse Practitioners:
• Master’s or Doctorate degree in Nursing from an accredited program
• Board Certification as a Family Nurse Practitioner (FNP)
• Active Connecticut APRN license (or eligibility)
For Physician Assistants:
• Master’s degree from an accredited Physician Assistant program
• NCCPA certification
• Active Connecticut Physician Assistant license (or eligibility)

• Strong interest or experience in outpatient primary care
• Excellent clinical judgment, communication, and patient care skills
• Open to experienced providers and new graduates
