We are seeking a dedicated and compassionate Home Health Aide (HHA) / Certified Nursing Assistant (CNA) to join our team of caregivers. About the Role:

• As an HHA/CNA, you will provide high-quality care and support to our clients, including assistance with daily living activities, wound care, and medication management.

• You will work closely with our interdisciplinary team to develop and implement personalized care plans that meet the unique needs of each client.

• You will maintain accurate records and communicate effectively with clients, families, and healthcare professionals.

Responsibilities:

• Provide routine care and assistance with daily living activities, including bathing, dressing, and grooming

• Assist with medication management, including administration and monitoring

• Maintain accurate records of client care and services provided

• Communicate effectively with clients, families, and healthcare professionals

• Work collaboratively with our interdisciplinary team to develop and implement personalized care plans

Requirements:

• Current certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA)

• Ability to lift, bend, and move safely in a fast-paced environment

• Strong communication and interpersonal skills

• Ability to work independently and as part of a team

• Compassionate and empathetic approach to client care
