This role focuses on helping residents obtain and maintain stable housing, build independent living skills, and access critical community resources.POSITION RESPONSIBILITIESProvide individualized case management services to tenants in supportive housing, including assessment, service planning, and ongoing supportConduct regular home visits and phone check-ins to monitor housing stability, safety, and overall well-being and provides appropriate documentation of all contactSupport residents in obtaining and maintaining entitlements and benefits (SSI/SSDI, Public Assistance, SNAP, Medicaid, etc.)Provide crisis intervention and stabilization support as neededConnect tenants to mental health, medical, vocational, and community-based servicesSupport development of independent living skills (ADLs) such as budgeting, cooking, hygiene, cleaning, and use of public transportationCollaborate with service providers and treatment teams to ensure coordinated careEnsures that room/apartments are clean for new tenants (may include cleaning, moving and setting up) & assists tenants with move in and move outs Monitor apartment conditions and report maintenance needs to ensure safe living environmentsAssists in the transportation of tenants to Mercy Haven programs and eventsResponsible for Agency compliance requirements by grants through site visits, data collection, and tracking systemParticipate in supervision, team meetings, and trainingsActs as task supervisor when interns are assigned to Supportive Housing ProgramAttends and participates in staff meetings and scheduled in-service training and/or additional educational opportunities as well as appropriate agency and interagency meetingsRotation of on-call dutiesAll other relevant duties as assignedPOSITION REQUIREMENTSHigh School Diploma or Equivalency with experience in human services requiredBachelor's Degree in Human Services or related field preferredExperience working with individuals with severe mental illness preferredValid NYS Driver's License with a safe driving record, access to a personal and reliable vehiclePhysical Requirements: Sits, stands, stoops, squats, walks, and moves intermittently during work hours, may push or pull 10-15 pounds daily, may push or pull up to 50 pounds occasionally, drive agency vehiclePOSITION RELATIONSHIPSReports to: Community Services CoordinatorSupervises: N/A*Training takes place Monday - Friday 8:30am - 4:30pm*SHIFT: Monday-Friday (8:30 am to 4:30 pm), with an on-call availability PAY RATE: $21.00 per hourLOCATION: Main Office in Islip Terrace, NYIf you're passionate about making a difference and contributing to our mission, we want to hear from you.
