Schedule: Weekdays, with occasional on-call/weekend requirement
Location: Grand Junction, Colorado
Sign-On Bonus: $6,000
Pay Range:
ASN: $35.02–$38.75/hour
BSN: $36.20–$40.05/hour

The Palliative Care Case Manager is responsible for the oversight and care of the patients in the Palliative Care programs of Transitions, Living with Cancer, and Journeys for the Grand Valley. This position is responsible for the development and implementation of the individualized patient care plans for patients/families experiencing serious illness. Assists patient in coordinating care through the healthcare system while focusing on comfort and symptom management needs for advancing disease. Discusses goals of care and assists with advance directives to navigate their care. Ensures a timely transition to hospice care when eligible.

What You Bring:
- Minimum 1–2 years of nursing experience preferred
- Experience in home health, palliative care, or acute care strongly preferred
- Current Colorado RN license maintained throughout employment
- Current CPR certification maintained throughout employment
- Valid Colorado driver’s license, reliable transportation, and proof of insurance
- Strong communication and interpersonal skills
- Ability to navigate emotional or high-stress situations with professionalism and compassion
- Experience with EHR systems and Microsoft Office applications
