Medical Scribe Hybrid (Remote and Skilled Nursing Facilities – Multiple Locations)

At ReNew Wound Care, we're redefining how wound care is delivered across post-acute settings. The Medical Scribe supports wound care providers in skilled nursing facilities through a hybrid model of in-person and remote documentation support. This role is responsible for accurately documenting patient encounters, wound assessments, procedures, and treatment plans while ensuring timely and compliant entries into the electronic medical record (EMR).
The Medical Scribe partners closely with providers and clinical teams to improve documentation efficiency, support regulatory and quality reporting requirements, and allow clinicians to focus on direct patient care. This position requires strong attention to detail, the ability to work independently in a remote environment, flexibility to provide onsite support during provider rounds as needed, and a collaborative approach to supporting team coverage needs.
Key Responsibilities:
- Accurately document patient visits, wound assessments, procedures, and treatment plans under the direction of the provider.
- Provide real-time documentation support during onsite provider rounding and remote documentation support as assigned.
- Ensure clinical documentation is complete,accurate, and entered into the EMRin accordance withorganizational standards, payer requirements, and regulatory guidelines.
- Support providers in capturing relevant clinical details, including wound progression, measurements, interventions, and follow-up recommendations.
- Assistwith chart preparation, review of patient information, and organization of documentation prior to patient encounters.
- Collaborate with providers and clinical teams to clarify documentation needs and ensure timely completion of medical records.
- Support quality reporting initiatives, including documentation requirements related to MIPS and other quality programs.
- Monitor documentation timelines to ensure completion and submission within requiredtimeframes.
- Maintain effective communication with skilled nursing facility staff, providers, and internal teams through approved communication platforms.
- Participate in cross-coverage support for fellow Medical Scribes during planned PTO, unexpected absences, or other operational needs to help ensure continuity of provider documentation support.
- Provide cross-coverage support based on availability, experience, assigned responsibilities, and operational requirements.
- Collaborate with team members to ensure smooth transitions and consistent documentation workflows during coverage periods.
- Maintain strict confidentiality of patient information andcomply withHIPAA regulations.
- Manage remote work responsibilities effectively, includingmaintainingreliable connectivity,appropriate workspacerequirements, andtimelycommunication.
- Travel to assigned skilled nursing facilities as needed to support in-person rounding.
- Perform other duties as assigned to support organizational needs.
Qualifications:
- High school diploma or equivalent required; associate or bachelor’s degree preferred.
- Prior experience as a medical scribe, clinical documentation specialist, or healthcare administrative support role preferred.
- Experience in wound care, geriatrics, skilled nursing, long-term care, or post-acute healthcare preferred.
- Strong knowledge of medical terminology, EMR systems, and healthcare documentation standards.
- Understanding ofquality reporting requirements, including MIPS documentation, preferred.
- Excellent written and verbal communication skills.
- Strong attention to detail and organizational skills.
- Ability to work independently, manage priorities, and meet deadlines in a hybrid work environment.
- Ability to collaborate effectively with team members and provide flexible support when needed.
- Professional demeanor with the ability to collaborate effectively with providers, facility staff, and clinical teams.
- Ability and willingness to travel to skilled nursing facilities as needed.
