Job Summary :We are seeking a compassionate and experienced Nurse Practitioner to provide high-quality primary care services to older adults in an assisted living/long-term care setting. This role focuses on delivering patient-centered care, managing chronic and acute conditions, and collaborating with an interdisciplinary team to improve health outcomes.
Key Responsibilities :Perform comprehensive health assessments and physical examinations.
Diagnose and manage acute and chronic medical conditions.
Develop and implement individualized treatment and care plans.
Order, interpret, and review laboratory and diagnostic tests.
Prescribe medications and treatments within the scope of practice.
Conduct medication reconciliation and ongoing medication management.
Collaborate with physicians, nurses, and other healthcare professionals to coordinate patient care.
Educate patients, families, and caregivers on disease prevention, treatment plans, and healthy lifestyle practices.
Maintain accurate and timely documentation in the electronic medical record (EMR).
Participate in quality improvement and patient safety initiatives.
Qualifications:Master's Degree or higher from an accredited Nurse Practitioner program.
Active New York State Nurse Practitioner or Physician license.
Active DEA registration.
Currently enrolled and credentialed with both Medicare and Medicaid.
Ability to practice independently in accordance with New York State requirements (NPs must have a minimum of 3,600 clinical practice hours).
Minimum of 3 years of clinical experience in primary care.
Strong clinical judgment, communication, and patient care skills.
Preferred Qualifications :Experience caring for geriatric patients.
Assisted living, skilled nursing, or long-term care experience.
Experience with electronic medical records (e.g., eClinicalWorks).
