Nurse Practitioner (Primary Care - Assisted Living)

Location: Bronx, NY
Job Type: Part-time (Onsite)
Schedule: 4-hour shifts, Monday-Friday (preferably 9:00 AM-1:00 PM or 1:00 PM-5:00 PM)
Practice Setting: Assisted Living / Geriatric Primary Care

Job Overview:
We are seeking a compassionate Nurse Practitioner to provide primary care services to residents in assisted living communities. This role involves managing acute and chronic conditions, developing care plans, collaborating with interdisciplinary teams, and delivering high-quality, patient-centered care. Key Responsibilities:

• Conduct patient assessments and physical exams

• Diagnose and manage acute and chronic conditions

• Develop individualized care plans

• Order and interpret labs and diagnostic tests

• Prescribe medications and monitor treatment plans

• Educate patients and families

• Maintain accurate EMR documentation

• Collaborate with the healthcare team to improve patient outcomes

Qualifications:

• Active New York State Nurse Practitioner license

• Active DEA registration

• Currently enrolled and credentialed with Medicare and Medicaid (required)

• Minimum 3 years of clinical experience

• Minimum 3,600 clinical practice hours for independent practice in NY

• Primary care experience required

• Geriatric, assisted living, or long-term care experience preferred

• Master's Degree in Nursing required
