Nurse Practitioner - Primary Care (Assisted Living Communities)

Location: Dutchess County, NY

We are seeking a compassionate and experienced Nurse Practitioner to provide high-quality primary care services to residents in assisted living communities across Port Jervis, NY and Middletown, NY. This full-time, onsite position offers an excellent work-life balance with no nights or weekends, working 30 hours per week over 4 days.

Key Responsibilities:

• Conduct comprehensive health assessments, physical exams, and medication reviews.

• Develop individualized care plans in collaboration with the interdisciplinary care team.

• Diagnose and manage acute and chronic medical conditions.

• Order, interpret, and evaluate laboratory and diagnostic tests.

• Prescribe medications within the Nurse Practitioner scope of practice.

• Educate residents and families on disease prevention, health maintenance, and wellness.

• Maintain accurate and timely documentation using eClinicalWorks (eCW).

• Participate in quality improvement initiatives to enhance patient outcomes.

• Build meaningful relationships with residents and their families while delivering patient-centered care.

• Master's Degree in Nursing from an accredited Nurse Practitioner program.

• Active New York State Nurse Practitioner license.

• Active DEA registration.

• Currently enrolled and credentialed with both Medicare and Medicaid (required).

• Minimum 3 years of clinical experience in primary care.

• Geriatric and/or assisted living/long-term care experience preferred.

• Ability to practice independently in accordance with New York State regulations.

• Strong communication, clinical, and patient care skills.

• $5,000 Sign-On / Retention Bonus

  • $1,000 on the first paycheck

  • $2,000 after 90 days

  • $2,000 after 120 days
