\nRequirements: Current state license as a Registered Nurse; Homecare Homebase Software Experience - Required; OASIS Experience - Required; Proof of current CPR certification - Required; Must have reliable transportation and insurance - Required; 1-2 years experience as a Registered Nurse in a clinical care setting - Required; 2+ years Home health experience - Required; Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program - Required.
\nResponsibilities:
\n- \n
- Assess home health and hospice patients in stable and emergent situations to identify physical, psychosocial, and environmental needs. \n
- Implement, develop, and evaluate the Plan of Care (POC) to ensure quality and continuity of care. \n
- Educate patients and families regarding care, medications, and safety, using appropriate teaching methods. \n
- Supervise and provide clinical directions to HHA/STNA/LVN to ensure quality service. \n
- Arrange referrals to appropriate agencies and monitor cases for compliance with payer requirements. \n
