Bridge Palliative Care CoordinatorLocation: Northern Utah/Southeast IdahoCompany: Ascend Home Health & HospicePosition Type: Full-TimeCompensation: $20–$25/hour DOEPosition SummaryAscend Home Health & Hospice is seeking a compassionate, organized, and relationship-focused Bridge Palliative Care Coordinator to support the growth and coordination of our palliative care program. This role serves as a key connection between patients, families, providers, referral sources, and the interdisciplinary care team to ensure patients receive exceptional supportive care focused on quality of life and symptom management.The ideal candidate is highly motivated, professional, detail-oriented, and passionate about serving patients with serious illness while helping guide them through available care options and resources.ResponsibilitiesCoordinate and support daily operations of the Bridge Palliative Care programCommunicate with referral sources, providers, patients, and families regarding services and eligibilityAssist with scheduling patient visits and provider coordinationTrack referrals, admissions, and patient follow-up needsMaintain accurate documentation and communication within the EMR systemSupport patient and family education regarding palliative care servicesCollaborate with clinical and administrative leadership to improve workflow and patient experienceBuild and maintain positive relationships with community partners and referral sourcesParticipate in program development and growth initiativesAssist with care coordination and transitions between care settings when appropriateQualificationsExperience in healthcare coordination, medical office operations, hospice, home health, or palliative care preferredStrong communication and customer service skillsExcellent organizational and multitasking abilitiesAbility to work independently and collaboratively within a team environmentComfortable working with patients and families facing serious illnessEMR experience preferredKnowledge of healthcare terminology and insurance processes is beneficialWhat We OfferCompetitive wage based on experienceMeaningful work that impacts patients and familiesSupportive and collaborative team environmentOpportunity to help grow an innovative palliative care programProfessional growth and leadership opportunitiesAt Ascend Home Health & Hospice, our mission is to Serve, Lift, and Unite through compassionate care and meaningful relationships.
