About the Role:
Coordinate daily pulmonary program activities, including patient assessments, treatment planning, and implementation of care plans
Collaborate with the interdisciplinary team to develop and implement effective treatment plans for patients with pulmonary conditions
Participate in patient care conferences to review patient progress and adjust care plans as needed
Monitor and report on patient outcomes, identifying opportunities for improvement and implementing changes as necessary
Develop and maintain relationships with healthcare providers, diagnostic centers, and other relevant entities to ensure continuity of care
Responsibilities:
Patient Care: Provide high-quality patient care, adhering to established standards and policies
Communication: Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions and optimal patient outcomes

Requirements:
Experience: 1-2 years in a health care facility
