PTA - FT

Provide treatment to assigned patients under supervision of Physical Therapist (PT) following established plan of care. Works collaboratively with supervising Physical Therapist & other healthcare professionals & staff to ensure patients reach maximum level of function.Essential Functions (Duties, Tasks & Responsibilities):Provide direct patient care following the Illinois State Practice Act (ISPA) & department procedures following plan of care under supervision of PT.Communicate regularly with PT on patient progress & any condition change.Provide patient &/or family education & home exercise program instruction.Maintain patient records including progress notes, discharges, notes to physicians & required prescriptions & signatures.
