Rated the BEST Senior Care Community in Wisconsin, we're seeking compassionate team members who embrace our core values: Respect, Commitment, Fulfillment, Empathy, and Fun!LPN: Licensed Practical Nurse Qualifications: Active Wisconsin Licensed Practical Nurse (LPN) licenseCurrent CPR certification Long-term care experience preferred Basic computer skillsAbility to push/pull up to 60 lbs. and occasionally lift up to 100 lbs.What You'll Do: Safely provide medications and treatments as prescribed by physicians, following accepted standards of practice and organizational policies.Maintain accurate, timely written and electronic records in compliance with legal, regulatory, and company requirements.Monitor and document residents' conditions, report changes to the RN and relevant team members, and collaborate in developing and implementing individualized care plans.Assist with admissions and readmissions, gather clinical data, support resident care programs by ensuring adherence to plans of care, and oversee CNA performance in these areas.Why Join Three Pillars?
