Serving with Compassion, Creativity, and Consistency. $24-26 an hour Position Summary: The Family Engagement Coordinator is a critical member of the residential and community residential program. The Family Engagement Coordinator's focus is on family voice and choice. The Family Engagement Coordinator is responsible for developing the after-care plan starting with the youth's placement on the dorm and focusing on permanency.  Responsibilities:Meet families in their homes and communitySchedule home visits with families, write visitation plans, schedule or provide transportationFocus on Permanency activities Ensure family voice and choice is evident in ICSP development, treatment plans goals, meetings, Trained in Child Adolescent Needs and Strengths Assessment (CANS) and gather information for Social Worker Gather information from family for 30 psychosocial Attend psychiatrist meeting if Social Worker is not available Provide casework services related to discharge planning and the youth's re-entry into the family and community Develops transition plans with and for the youth, including education/vocational/life skills.
