Salary Range: $72,000 - $79,000 / year

Physical Therapy Assistant Position Description:

Provides physical therapy treatment modalities in accord with the patient plan of care established by the supervising physical therapist.

Responsibilities:

1. Provides direct care to treat the medical/surgical condition through the use of accepted physical therapy modalities as assigned by the physical therapist.
2. Instructs the patient/family and home health aide (and others) in the home exercise program and other aspects of patient care as directed by the physical therapist and documents the response to instruction.
3. Provides written home exercise program to patient (and others) as needed and also provides a copy for the patient’s record.
4. Continually monitors the patient's condition and promptly reports any changes to the physical therapist and clinical manager
5. Documents all clinical visits and patient's response to therapy interventions in the EMR.
6. Submits completed clinical documentation in accordance with agency policies.
7. Participates in patient care conferences.

Qualifications:

1. NYS Professional License and current registration to practice as a Physical Therapy Assistant.      
2. Minimum of two (2) years direct clinical experience.
3. Home care experience preferred.
4. Automobile in good working order and auto insurance in the amount specified by VNSW.
5. Valid driver’s license.
6. Knowledge of HCHB Clinical Software preferred.
7. Must maintain CEU requirements.
8. Professional Liability Insurance in the amount specified by VNSW.
