PRN NOC LPN/RN – Long-Term Care Job DescriptionPosition TitlePRN NOC Shift LPN/RN (Licensed Practical Nurse / Registered Nurse)Position SummaryThe PRN NOC Shift LPN/RN provides skilled nursing care to residents during overnight hours in a long-term care facility. This position is responsible for assessing residents, administering medications and treatments, monitoring changes in condition, responding to emergencies, and ensuring resident safety and comfort throughout the night shift. The nurse works collaboratively with nursing assistants, healthcare providers, residents, and families to deliver quality care in accordance with facility policies and state and federal regulations.SchedulePRN (as needed)Night shift / NOC shift  (6:00 PM – 6:00 AM)Weekends and holidays as neededEssential Duties and ResponsibilitiesProvide direct nursing care to residents in a long-term care settingAdminister medications, treatments, and injections as ordered by physiciansMonitor residents for changes in condition and report concerns appropriatelyComplete nursing assessments and documentation accurately and timelyRespond to resident call lights and medical emergencies during overnight hoursSupervise and support Certified Nursing Assistants (CNAs)Ensure compliance with infection control, safety, and resident care policiesMaintain accurate medical records and shift reportsCommunicate effectively with residents, families, physicians, and interdisciplinary team membersAssist with admissions, discharges, and transfers as neededPromote resident dignity, comfort, and quality of lifeQualificationsCurrent LPN or RN license in the state of employmentCurrent CPR/BLS certification preferred or requiredPrevious long-term care or skilled nursing experience preferredAbility to work independently and make sound clinical decisionsStrong communication and organizational skillsAbility to handle emergency situations calmly and professionallyPhysical RequirementsAbility to stand, walk, bend, and lift throughout shiftAbility to assist residents with mobility and transfersAbility to lift up to 50 pounds with assistance when necessaryWork EnvironmentLong-term care/skilled nursing facilityFast-paced healthcare environmentFrequent interaction with elderly or medically complex residentsSkills and CompetenciesMedication administrationPatient assessment and monitoringTime management and multitaskingTeam collaborationCompassionate resident-centered careDocumentation and charting proficiencyEqual Opportunity StatementEstherville Community Care Center is an equal opportunity employer and is committed to creating an inclusive environment for all employees
