Job Summary
We are seeking a Licensed Practical Nurse to join our team. In this role, you will primarily be creating and managing a plan of care for Medicaid clients. 

Responsibilities 

• Coordinate with the team to assess, plan, implement, or evaluate patient care plans 
• Document any changes in patient conditions and report on changes
• Nurse will perform Quarterly, annually or every six-month visit with patients for routine assessment and routine recertification.
• Maintain accurate records of patient care and discuss observations with nurse manager
• Assist with admissions, discharges, and patient assessments
• Maintain accurate and timely medical records
• Support and educate patients, caregivers and families regarding care plans.

Qualifications

• Currently Licensed Practical Nurse (LPN) in state of employment 
• Previous experience as an LPN is preferred
• Excellent interpersonal and client care skills
• Valid driver’s license 
• First aid/CPR certification required 
• Prior experience in long-term care, assisted living, rehabilitation, home health, or clinical settings
