Your responsibilities include:Conducting initial and ongoing assessments to evaluate patient functionCollaborating with physicians to develop or revise individualized plans of carePreparing clinical and progress notes in accordance with regulatory standardsProviding therapeutic treatments tailored to each patient's needsConsulting with families and agency staff to support patient goalsContinuously evaluating treatment effectiveness and updating plans as neededCoordinating services with other home health team membersSupervising physical therapy assistants and home health aides when appropriateAssessing equipment needs to ensure optimal patient outcomesFollowing CMS documentation and billing requirements, including functional reassessmentsPracticing infection control and universal precautionsReporting suspected abuse according to state and federal guidelinesAssisting with new‑hire orientation and participating in training activitiesYou take pride in delivering high‑quality care that directly improves patients' lives.QualificationsSuccessful completion of an accredited physical therapy education programCurrent Oklahoma PT license in good standingAt least 1 year of physical therapy experienceCPR certification (or ability to obtain within 60 days)Ability to meet the physical demands of the jobWe're looking for someone who communicates clearly, stays organized, manages time well, and maintains a positive, professional demeanor.Work ScheduleFlexible schedule, Monday-Friday, 8:00 AM - 5:00 PMReady to Join Our Team?Our application process is quick and simple.
