• Conduct thorough assessments of patients' medical, physical, and social needs, using the Minimum Data Set (MDS) to inform care plans

• Develop and implement individualized care plans that address patients' unique needs and goals

• Collaborate with interdisciplinary teams to ensure comprehensive care and address any concerns or issues that arise

• Monitor patients' progress and adjust care plans as needed to ensure optimal outcomes

• Maintain accurate and detailed records of patient assessments, care plans, and progress
