Description
American Traveler is seeking a Home Health Physical Therapist with at least 1 year of home health experience and proficiency in OASIS documentation.
Details
- Home health setting
- Day shift schedule, 5x8-hour shifts from 8:00 AM to 4:30 PM
- 13-week assignment length
- Floating may be requested based on facility needs
- Requires proficiency with OASIS documentation
- Epic EMR experience required
Requirements
- Minimum 1 year of home health experience required
- OASIS experience required
- BLS certification required
- Manual therapy certification preferred
- Epic charting experience required
- License required if applicable
- Specialty experience within the last year required
- Skills in home exercise programs, adaptive devices, gait training, and assistive devices
- Experience with orthopedic, cardiopulmonary, and neurologic patient populations preferred
- Familiarity with equipment such as gait belts, walkers, hospital beds, and slings
Additional Information
- Responsibilities include patient assessment, evaluation, treatment planning, and discharge documentation
- Duties may include ordering supplies and scheduling patient appointments
- Business casual attire or solid color scrubs permitted
- Local candidates accepted at the same pay rate
- Candidates should keep requested time off to a minimum during the assignment, ideally no more than 7 days total
- Previous employees must have a gap of at least 6 months since perm employment before being eligible for travel assignments
