- Active Oregon Physical Therapist License required
- Minimum 1 year of recent Skilled Home Health experience
- OASIS experience required
- BCLS certification required
- First-time travelers accepted
- Must have reliable transportation
- COVID-19 primary vaccination series plus one booster required
