Physical Therapist – Home Health

Key Responsibilities- Provide direct physical therapy services to home health patients in accordance with each patient's individualized plan of care
- Collaborate with interdisciplinary team members to develop and update comprehensive patient care plans
- Evaluate and recommend rehabilitation equipment including splints, orthoses, prostheses, wheelchairs, and ADL equipment
- Apply appropriate therapeutic modalities as indicated by patient need and clinical assessment
- Complete and maintain accurate clinical documentation, including OASIS assessments, in a timely manner
- Travel independently to patient homes within the designated service area
- Active Massachusetts Physical Therapist license (must be provided at time of submission)
- Current Basic Life Support (BLS) certification through the American Heart Association
- Certified Home Health experience with demonstrated proficiency in OASIS documentation (required and non-negotiable)
- Minimum of 2 years of acute nursing care (hospital-based) and home care experience
- Valid driver's license and personal vehicle required for patient home visits
- Ability to work independently with minimal supervision
- Additional acute care experience beyond minimum requirements
- Experience working in a teaching hospital or large health system environment
