Job Summary:
Speech Language Pathologist (SLP) in Home Health located in Oregon. This role involves providing speech therapy services to patients with speech, language, or swallowing disorders in a home health setting. The position requires delivering personalized patient care, assessing needs, and modifying treatment plans to improve patient outcomes.
Job Details:
- Schedule: Days, 8 hours shifts, minimum of 40 hours per week.
- Required Qualifications: Master's Degree or foreign equivalent, Speech Pathologist licensure in the state of practice, Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per organization policy, valid drivers license, and must be at least 21 years of age or older.
- Preferred Qualifications: American Speech-Language and Hearing Association (ASHA) Certificate of Clinical Competence (CCC).
- Essential Functions: Assesses and documents speech therapy data related to patient assessments, care needs, interventions, and outcomes. Identifies changes and trends in physical and emotional parameters. Reviews treatment plans and assesses individual performances to modify or create new programs. Maintains documentation as required by law, establishment policy, and administrative regulation. Instructs patients on ways to monitor speech and practice new skills. Performs other job-related duties as assigned.
