Job Opportunity – Physical Therapist, Home Health – Hood River, OR 97031

Start Date: September 7, 2026

Location: Hood River, Oregon and surrounding areas

Schedule: Day shift, 8-hour shifts (08:00 - 16:30), Monday through Friday. Patient visits average up to 6 per day with a standard of 30 productivity points. 1:1 patient ratio during home visits.

Requirements:

• Active Physical Therapist license in both Oregon and Washington (both required)
• Minimum 1 year of skilled home health experience required
• OASIS documentation experience required
• Basic Life Support (BLS) certification
• Experience with EPIC charting system preferred (EPIC Remote Client for Home Health and Rover phone app)
• Reliable personal transportation required
• COVID-19 primary vaccination series plus one booster required
• Two professional references covering 1 year within the last 3 years
