Pride Health is actively hiring a Physical Therapist (PT) to join our client's growing Home Health team.

Compensation:

Travel Pay Package: Up to $2,700/week gross (includes weekly non-taxable stipend)*

Location: Hopkinsville, KY, 42240

Schedule details (hours per day/week): 32

Productivity expectations: 30

On Call/Weekend Requirement: None
Coverage Areas: Christian - Trigg - Caldwell - Lyon
Shift schedule (days, start & end times): Mon- Friday 8-430
Will Milage be reimbursed between visits, from first patient to last patient (Y/N)? yes

Key Responsibilities:

• Evaluate and develop Plan of Care that will improve physical functioning to assist the patient to meeting and exceeding goals if possible to remain safe and in the home.
• Conduct comprehensive home health physical therapy services, including Start of Care (SOC), initial evaluations, routine revisits, re-evaluations, and discharge assessments, while developing individualized treatment plans, improving patients' functional mobility, and meeting a productivity expectation of 30 points.
• Deliver evidence-based therapy interventions, monitor and document patient progress accurately in Homecare Homebase (HCHB)/PointCare, collaborate with physicians and the interdisciplinary team, educate patients and caregivers, and ensure compliance with Medicare, agency, and home health documentation standards across the assigned coverage areas.
