- Proof of auto insurance and a clean driving record
- Active American Heart Association Basic Life Support (BLS) certification
- Verified work history and skills relevant to occupational therapy and home health, current within the past year
- Two supervisory reference forms completed within the last two years
- Perm address must be included in the profile
- All scheduling requests must be made at the time of submission; no requests can be accommodated after offer or interview

- No holiday or on-call coverage required
- Dress code: Olive green scrubs
- Typical daily mileage: approximately 35-50 miles, paid at current Federal Rate for travel to and from the main office
