Job Summary:
Physical Therapist (PT) specializing in Home Health (HH) services in a home setting. The position involves providing treatments to patients in their homes, assisted living facilities, senior apartments, and board and care homes. The role requires working within a dynamic care team to deliver quality patient care across a designated territory.
Job Details:
- Schedule: Days, 8-hour shifts, Monday through Friday, with the expectation to complete work at the end of each shift. Work is scheduled for 40 hours per week, with one weekend per month required (not mandatory). Holiday work may be required with flexibility for time off if requested.
- Required Qualifications: Valid California Physical Therapist (PT) license, BLS (American Heart Association) certification, National Provider Identifier (NPI), current driver's license, and proof of automobile insurance. Must possess at least 2 years of experience in a healthcare setting, including home health environment with OASIS experience. Good oral and written communication skills, organizational skills, and the ability to work in home environments that may be cramped, hot/cold, or cluttered. Experience with Electronic Homecare Home Base (HCHB) is preferred.
- Preferred Qualifications: Experience with community/home health services and prior knowledge of Homecare Homebase (HCHB) documentation system.
- Additional Information: The role entails supervising up to two Physical Therapist Assistants or Certified Occupational Therapist Assistants. Visits typically include Start of Care, evaluations, and treatment plans, averaging 3-5 patients daily depending on the visit type. The territory serviced includes regions primarily served by Mercy area hospitals and primary care providers. The position involves travel within designated territories with mileage reimbursed at the GSA rate with prior approval. Orientation includes a three-week online learning and field precepting process with solo work expected by the end of week two or three.
