Out Patient Physical Therapist
Shift 7a - 4p or 8a - 5p
Outpatient Hours 8a - 5p Mon-Thu & 8a - 1p on Fri.
Weekend Shifts Required
AZ License Required
EMR ReDoc & MedHost
PT would see about 9-11 patients per day
Currently, 2 Out Patient PT's and 3 In Patient PT's on Staff
Travelers must show proof of residence greater than 50 miles from facility
& have not worked perm. Staff for CHS with in the last 12months
