Physical Therapist
Job Summary: The Physical Therapist is responsible for providing physical therapy services to patients in their homes, addressing physical impairments, functional limitations, or disabilities that result from injury, disease, or other causes. The patient demographic may range from infancy to over 100 years of age. Collaboration with patients, families, physicians, and other healthcare professionals is essential to identify patient needs and coordinate appropriate services. Documentation and communication must be thorough and timely, adhering to agency policies and standards of practice.
Responsibilities:- Provide physical therapy services to patients in their place of residence.
- Assess patient needs and develop individualized care plans.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care.
- Maintain accurate electronic documentation in accordance with agency standards.
- Engage in ongoing education and training to stay current with industry practices.
- Must hold a valid Physical Therapist Professional license in Massachusetts.
- Minimum of 1 year of home care experience is required.
- At least 1 year of experience in case management within a certified home health agency.
- Weekend rotations required approximately every 6 weeks.
- Ability to travel to assigned cases as needed.
Please note: Experience in hospice, private care, or as a standalone case manager does not fulfill the certified home health care management experience requirement. Only roles within a certified home health agency will count toward the 1-year experience requirement.
Start Date: The start date will be determined based on credentialing clearance.
