Job Details
- Schedule: 8-hour days, minimum of 40 hours per week.
- Required Qualifications: 1 year of skilled home health experience, Oregon license, Washington license, BLS certification, and successful completion of Oasis documentation.
- Preferred Qualifications: Experience with EPIC charting system.
- Certifications: BLS required.
- Additional Information: Traveler must have reliable transportation; expected to visit up to six patients per day within a maximum radius of approximately 60 miles. The position involves minimal drive and windshield time, with occasional local travel within a 50-75 mile radius. The facility does not accommodate block schedules, and flexible start dates are acceptable. Patient diagnoses include dementia, Parkinson's disease, ALS, MS, orthopedic post-op, and post-stroke rehabilitation.
