Position Summary
We are seeking a licensed Physical Therapist (PT) to provide high-quality home health rehabilitation services. In this role, you will evaluate patients, develop individualized treatment plans, and deliver evidence-based therapy to improve mobility, restore function, and enhance independence while collaborating with an interdisciplinary care team.
Key Responsibilities
- Perform comprehensive physical therapy evaluations and assessments.
- Develop, implement, and update individualized Plans of Care (POC).
- Provide therapeutic interventions for orthopedic, neurological, and neuromuscular conditions.
- Monitor patient progress and modify treatment plans as needed.
- Educate patients and caregivers on therapeutic exercises, home safety, mobility techniques, and use of assistive devices.
- Recommend and train patients on walkers, wheelchairs, canes, braces, prosthetics, and other durable medical equipment (DME).
- Complete accurate and timely clinical documentation, progress notes, physician orders, and care plans.
- Collaborate with physicians, nurses, and the interdisciplinary team to ensure coordinated patient care.
- Follow infection prevention, safety, and regulatory guidelines during patient care.
- Travel to patient homes within the assigned coverage area.
Required Qualifications
- Active Physical Therapist (PT) license in the practicing state.
- Bachelor's degree in Physical Therapy from an APTA-accredited program.
- 1–2 years of experience as a Physical Therapist in acute care, rehabilitation, or a related clinical setting.
- Current CPR/BLS certification.
- Valid driver's license, reliable transportation, and current auto insurance.
