Physical Therapist – Home Health (Travel Contract)
LocationStockton, CA (Central Valley – includes surrounding areas such as Modesto)
Schedule / Job TypeContract | Days | Full-Time | Onsite / Field-Based | 5x8-Hour Shifts (8:00 AM – 5:00 PM) | Schedule options include Tuesday–Saturday, Wednesday–Sunday, or Monday–Friday
Key Responsibilities- Provide physical therapy services to patients in a home health setting across an assigned geographic territory
- Complete OASIS assessments accurately and in a timely manner
- Maintain timely clinical documentation in accordance with regulatory and organizational standards
- Manage an independent caseload with travel between patient homes within the assigned coverage area
- Participate in on-call rotation as required
- Complete required orientation modules, which are billable and conducted during orientation
- Active California Physical Therapist license (candidates with a pending CA license who have applied at time of submission will be considered)
- BLS certification through the American Heart Association (AHA) required
- OASIS experience required
- Active, valid driver's license and personal reliable transportation required
- Minimum of 1 year of home health experience
- Timely communication and documentation are mandatory
- California license in hand at time of submission preferred
- Candidates without home health experience will be considered on a case-by-case basis
- Guaranteed hours structure based on completed visits, providing predictable workload expectations
- Mileage reimbursement between patients at the IRS standard rate
