Job Summary:
Physical Therapist (PT) Home Health in Hilo, Hawaii. This position involves providing rehabilitation services within a Skilled Nursing Facility (SNF) and/or Intermediate Care Facility (ICF). The role encompasses performing evaluations, treatments, and clinical services in accordance with licensure, discipline, and scope of practice, ensuring compliance with state and federal regulations. A Hawaii State license in good standing is required prior to start date.
Job Details:
- Location: Hilo, Hawaii
- Shift Schedule: Rotating shift; generally scheduled five (5) days per week, likely including one weekend day; possible floating to other units or facilities based on census and staffing needs.
- Required Qualifications:
- Minimum two (2) years of experience in the SNF/ICF setting preferred.
- Current Basic Life Support (BLS) certification.
- Current State of Hawaii license in good standing appropriate to the discipline.
- Hawaii License must be obtained before the start of the assignment.
- Additional Information:
- Traveler not currently possessing a Hawaii license must apply upon acceptance of the offer, and must have the license by the start of the assignment.
- Must arrive the Thursday before the start date to attend the employee health appointment on Friday.
- Responsibilities include developing and implementing individualized treatment plans, executing evidence-based therapeutic interventions, and educating residents, families, and caregivers.
- Supervision of therapy assistants may be applicable.
- Orientation to the rehabilitation department, electronic medical records, and facility workflows will be provided.
