Description
American Traveler is seeking a Physical Therapist for a Home Health role requiring a valid driver's license and reliable transportation to travel to patients' homes.
Details
- Home health and hospice setting serving primarily geriatric patients
- 5 days/week, 40 hours/week schedule on AM shift
- 8-hour shifts with 30-minute meal break
- No on-call requirements
- Work performed in patient homes, office settings, and community environments
Requirements
- License required if applicable
- Current driver's license and reliable vehicle required for travel to patient homes
- 2+ years of previous Physical Therapist experience strongly preferred
- Ability to perform comprehensive physical therapy assessments and develop individualized treatment plans
Additional Information
- Supervise and coordinate care with Physical Therapist Assistants
- Provide therapeutic interventions focused on mobility, strength, pain reduction, and functional independence
- Collaborate with an interdisciplinary care team on treatment and discharge planning
- Educate patients and caregivers on safe mobility techniques, equipment use, and home-exercise programs
- Care is delivered under medical direction and evaluated in accordance with organizational policies and standards of performance
