Description
American Traveler is seeking a Home Health Physical Therapist with a valid CA PT License and at least 1 year of Home Health PT experience.
Details
- Home Health setting with visits made in patients' homes
- Day shift schedule, 5x8-hour shifts from 8:00 AM to 5:00 PM
- Caseload of approximately 4-5 patients per day, based on 5 weighted visits per day
- 13-week assignment length
Requirements
- Current CA PT License required (license required if applicable)
- Minimum 1 year of experience as a Home Health Physical Therapist required
- Valid driver's license and auto insurance required
- BLS certification preferred
- Experience with home health documentation including OASIS, Plan of Care, and RUG Levels preferred
- Skilled in orthopedic, neurologic, and geriatric patient care preferred
Additional Information
- Responsibilities include patient assessment, home exercise program development, and patient/family education
- Treats a variety of conditions including orthopedic injuries, neurologic disorders, and post-surgical rehabilitation
- Utilizes home health equipment such as gait belts, walkers, Hoyer lifts, and specialty beds as needed
- Maximum RTO of 7 days per contract, with only 1 holiday request approved per contract
- Holiday coverage includes New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, and Christmas Day
- Accepts local candidates within a 50-mile radius at the local rate
- Returning staff or travelers must have a separation period before reapplying
