Key Responsibilities- Provide skilled physical therapy services to patients in a Home Health and Hospice setting
- Conduct patient evaluations and develop individualized treatment plans
- Deliver therapeutic interventions aimed at improving patient mobility, function, and quality of life
- Coordinate care with interdisciplinary team members to ensure comprehensive patient support
- Maintain accurate and timely clinical documentation in accordance with regulatory and organizational standards
- Active Physical Therapist licensure in the state of Washington (or eligibility to obtain)
- Previous Home Health physical therapy experience
- Strong clinical assessment and documentation skills
- Ability to work independently in a home-based care environment
- Experience with Home Base (electronic medical record or scheduling platform)
- Familiarity with Hospice care settings and interdisciplinary care coordination
- Work in a supportive Home Health Hospice team environment in a region known for its outdoor recreation, natural beauty, and over 300 days of sunshine annually
- Lodging options are available in the local area to support traveling clinicians during their assignment
