Job Summary:
Physical Therapist (PT) specializing in Home Health services is sought for a position serving a primarily rural region across four counties. This role involves providing patient care in a home health setting, with responsibilities including conducting Oasis Assessments and Start of Care cases. The position requires a current license to be submitted upon application. The assignment is scheduled to begin on 08/17/2026, with work hours from 8:00 am to 5:00 pm, Monday through Friday.
Job Details:
- Location: 401 Sawyer Road, Kendallville, Indiana, 46755
- Schedule: Day shift, Monday through Friday, 8:00 am to 5:00 pm, minimum of 40 hours per week
- Required Qualifications: Valid license to practice as a physical therapist; BLS (AHA) certification; at least 1 year of home health experience; willingness to wait for license submission
- Preferred Qualifications: Experience with Homecare Homebase (HCHB) charting system; home health experience with home Base
- Float: Not applicable
- Additional Information: Serves four counties with a focus on elderly patient care including falls and fractures; uses point system equivalent to 4-6 patient visits daily; tablet technology for tracking visits and time; business casual or Hunter green scrubs with white or black top preferred. The facility allows cancellation of up to 4 shifts per 13-week assignment or 3 shifts for shorter assignments.
