Home Care Physical Therapist

Key Responsibilities- Assess patients' conditions and develop individualized plans of care to maximize mobility and independence
- Collaborate with an interdisciplinary team to ensure patients' needs are met
- Perform standard therapy measurement techniques including ROM, strength, and balance testing
- Utilize a variety of therapeutic modalities to maximize care provided to patients in a home setting
- Provide case management responsibilities as part of the home health care team
- Active Vermont Physical Therapist licensure
- Home Health experience required
- OASIS experience required
- Ability to provide one-on-one therapy care in a patient home setting
- Experience with McKesson/Netsmart EMR system strongly preferred
- Prior case management experience in a home health environment
