Key Responsibilities- Provide physical therapy services in a hospital-based outpatient setting
- Deliver inpatient acute and swing bed physical therapy care at a Critical Access Hospital
- Evaluate patients, develop individualized treatment plans, and monitor progress toward goals
- Collaborate with interdisciplinary care teams to ensure coordinated patient outcomes
- Maintain accurate and timely clinical documentation in accordance with facility standards
- Minimum 2 years of current Physical Therapy experience (new graduates will not be considered)
- Outpatient Physical Therapy experience required
- Active New Hampshire Physical Therapist license (must be current and in hand at time of submission)
- Active American Heart Association (AHA) Basic Life Support (BLS) certification (must be current and in hand at time of submission)
- Completed Physical Therapy skills checklist required
- One supervisory signed reference from within the past year of employment required
- Inpatient Physical Therapy experience highly preferred
- Experience working in a Critical Access Hospital or rural healthcare environment
